Para esta unidad se requieren conocimientos y habilidades de aritm\u00e9tica b\u00e1sica.<\/p>\n<h2>COMPETENCIA<\/h2>\n<p>Adquirir las habilidades y destrezas relacionadas con la valoraci\u00f3n nutricional para realizar valoraciones y calcular los requerimientos nutricionales en base a las necesidades identificadas en las personas, en un marco de probidad y eficiencia.<\/p>\n<h2>CONTENIDO<\/h2>\n<h3>2. Componentes del gasto energ\u00e9tico.<\/p>\n<p><strong>Metabolismo basal<\/strong>: El metabolismo basal representa la m\u00ednima cantidad de energ\u00eda que se requiere para mantener el organismo despierto y en reposo. En el estado de reposo, la energ\u00eda se gasta en las actividades mec\u00e1nicas necesarias para conservar los procesos de la vida, como respiraci\u00f3n y circulaci\u00f3n, s\u00edntesis de constituyentes org\u00e1nicos, bombeo de iones a trav\u00e9s de las membranas, conservaci\u00f3n de la temperatura corporal. No incluye las necesidades de energ\u00eda para la actividad f\u00edsica o digesti\u00f3n de alimentos.<\/p>\n<p><strong>Efecto t\u00e9rmico de los alimentos<\/strong>: Representa la cantidad de energ\u00eda que utiliza el organismo durante la digesti\u00f3n, absorci\u00f3n, metabolismo y almacenamiento de nutrimentos que proporcionan energ\u00eda. Esto eleva la temperatura corporal por varias horas despu\u00e9s de comer. La actividad f\u00edsica representa gasto de energ\u00eda.<\/p>\n<p><strong>Actividad f\u00edsica<\/strong>: Representa la energ\u00eda gastada por el ejercicio voluntario y el ejercicio debido a la actividad involuntaria como escalofr\u00edos, movimientos y control postural. El requerimiento diario de energ\u00eda se estima al sumar los siguientes componentes:<\/p>\n<ol>\n<li>El metabolismo basal.<\/li>\n<li>El efecto t\u00e9rmico de los alimentos.<\/li>\n<li>La actividad f\u00edsica.<\/li>\n<\/ol>\n<p>Estos dos primeros factores nos dan el gasto energ\u00e9tico en reposo.<\/p>\n<h4>M\u00e9todo FAO \/ OMS \/ UNU (1985):<\/h4>\n<p>Un comit\u00e9 de expertos de la FAO \/ OMS \/ UNU (Universidad de las Naciones Unidas), propuso las siguientes ecuaciones para calcular el gasto energ\u00e9tico en reposo (lo cual incluye el metabolismo basal y el efecto t\u00e9rmico de los alimentos).<\/p>\n<p>Ecuaciones para estimar el GER de sujetos sanos. Report of a Joint FAO\/WHO\/UNU Expert Consultation.<\/p>\n<p>Estas ecuaciones nos permiten calcular el gasto energ\u00e9tico en reposo de una persona, a partir su edad y sexo, utilizando su peso adecuado (de acuerdo al \u00edndice de masa corporal normal, que en el adulto se considera de 18.5 a 24.9). Por \u00faltimo, se simplific\u00f3 la forma de estimar el gasto energ\u00e9tico por actividad f\u00edsica. Se expres\u00f3 el gasto energ\u00e9tico por actividad como m\u00faltiplos del GER y clasific\u00f3 a las diversas actividades. Recordando que una de las caracter\u00edsticas de la dieta correcta es el equilibrio de los macronutrimentos, o sea, de las prote\u00ednas, grasas e hidratos de carbono, se ha recomendado el porcentaje con el cu\u00e1l cada uno de estos nutrimentos debe participar en la composici\u00f3n de una dieta normal:<\/p>\n<ul>\n<li>Los hidratos de carbono (HC) deben aportar del 55 al 65% del total de la energ\u00eda.<\/li>\n<li>Los l\u00edpidos o grasas deben aportar del 20 al 30% de la energ\u00eda.<\/li>\n<li>Las prote\u00ednas deben aportar del 10 al 15% de la energ\u00eda.<\/li>\n<\/ul>\n<p>Un c\u00e1lculo recomendable de prote\u00ednas es de aproximadamente 0.8 g de prote\u00ednas por kg de peso corporal. Equivalentes recomendados por grupos de alimentos.<\/p>\n<p>Posteriormente, se multiplicar\u00e1 el n\u00famero de equivalentes establecido por el valor unitario de energ\u00eda (kcal), g de prote\u00ednas, g de grasas y g de hidratos de carbono de cada grupo de alimentos.<\/p>\n<p><strong>Distribuci\u00f3n de los equivalentes por grupos de alimentos:<\/strong><\/p>\n<p>Los c\u00e1lculos de energ\u00eda (en kcal), prote\u00ednas (en g), grasas (en g) e hidratos de carbono (en g) se van sumando en la tabla de valores de equivalentes, hasta lograr que los valores de la energ\u00eda y <strong>macronutrimentos <\/strong>se asemejan a lo calculado en el paso anterior. Es importante recordar que una adecuaci\u00f3n del 100% en estos c\u00e1lculos es imposible, por lo que un margen de error de <u>+<\/u> 10% es aceptable.<\/p>\n<p><strong>Distribuci\u00f3n de equivalentes por tiempos de comida:<\/strong><\/p>\n<p>Distribuir el n\u00famero de equivalentes de cada grupo de alimentos para cada tiempo de comida. Esto se realizar\u00e1, si es posible, con el paciente para poder elaborar una dieta de acuerdo con sus gustos y posibilidades. La evaluaci\u00f3n del estado de nutrici\u00f3n debe formar parte del examen rutinario de la persona sana y es una parte importante de la exploraci\u00f3n cl\u00ednica del paciente enfermo. M\u00e9xico, 2008.<\/li>\n<\/ul>\n<h2>ANEXOS<\/h2>\n<p><strong>Sistema Mexicano de Alimentos Equivalentes<\/strong><\/p>\n<p>Los alimentos que se presentan en los grupos de equivalentes est\u00e1n considerados con su peso neto, es decir, sin c\u00e1scara, semilla o hueso.\u00a0 Los alimentos que por lo general se consumen cocidos, tienen el peso considerado en cocido o guisado, pues, al cocinarse algunos de ellos absorben agua (cereales y leguminosas), en tanto que otros la pierden (tejidos animales). La gu\u00eda de alimentaci\u00f3n se elabora con base en las necesidades energ\u00e9ticas y de otros nutrimentos previamente calculada, en las costumbres y h\u00e1bitos alimentarios, el nivel socioecon\u00f3mico, el clima, los alimentos disponibles y otros factores. Por supuesto, se debe considerar el n\u00famero de comidas que cada persona acostumbre o le haya sido recomendado de acuerdo a su padecimiento.<\/p>\n<p>Es importante elaborar en cada caso un ejemplo de men\u00fa adecuado a las caracter\u00edsticas propias del individuo y no prescribir la misma dieta a todos los pacientes. El empleo del sistema de equivalentes no requiere de comidas especiales ni de formas de preparaci\u00f3n diferentes a las habituales en el hogar y puede considerarse como un medio para la variaci\u00f3n razonable de la comida ordinaria.
